Traffic Accident Reporting

Note: the Massachusetts Registry of Motor Vehicles' Driving Manual has a section on what to do when you've been in an accident.

Within five days of the accident, you will need to file a Commonwealth of Massachusetts Operator's Accident Report when the accident results in personal injury or death, when property damage exceeds $1,000, when one of the vehicles involved has fled the scene or when you have any doubt as to the above.

When filling out an accident report you are required to provide the following information (please print clearly):

*This information must be provided to the RMV if it has been provided to you as a result of any paperwork exchange at the time of the accident or otherwise. If this information is not known to you, please write UNKNOWN in the applicable section.

Keep in mind that the release of this information by the Registry of Motor Vehicles is restricted by Federal Law, so do not call or visit an RMV Office to request this information.

 Mass RMV Address Change

This form is used to inform the RMV of a change of address.

Please read these instructions before downloading this form.

  1. Complete this form and mail it in an envelope to the following address or return it to any RMV branch office.

    RMV, Attn: Address Change, PO Box 199106, Boston, MA 02119-9106

  2. You will receive a letter of confirmation in the mail within seven (7) days. This letter will contain four (4) preprinted labels for you to attach to your license and registration(s).

  3. If you have more than one vehicle registered in your name, enter all plate numbers.

  4. Please make sure all information is correct and legible.

Please note, if you want your change of address to affect your voter registration, you must submit an application for change of information at an RMV licensing location which is found at any RMV branch office or License Express location. Or you may visit your new city/town hall, or contact the Secretary of Commonwealth site for information.

Complete all required sections.  Enclose a check or money order and mail to the address on the form.
